Financial Reporting

All annual and final financial reports must use Federal Financial Report, SF-425, available on Grants.gov's . Use the NHPRC Financial Reporting Instructions to fill out the form.


Narrative Reporting


Payment Instructions

To request grant funds, you must use the Request for Advance or Reimbursement, Standard Form 270. A link to this form and detailed instructions can be found on our Payment Instructions for Grantees page.


Extension Requests

After discussing your request with your Program Officer, you will need to submit a formal Extension Request.

    NHPRC grantees and sub-recipients must comply with the requirements of the which requires that they purchase only American-made equipment and products in expending grant funds.
